The image disappear when you rotate over a certain angle.  Are
you initializing everything (I checked your rotation matrix, it
is OK).

The commented out perspective matrix has some values flipped
(the (0,0) and (1,1) fields are incorrect).  If this was your
earlier attempt, then the problem is explained.
