Enter your last name (or family name) followed by a coma and then your first name. Optional: you may include your middle name following your first name.
Example: Winston, Patrick H. or Winston, Patrick Henry
Enter your nick name or the name that you prefer to be called.
Example: Pat
Enter your course of study.
Example: If your are in EECS enter 6.
Enter your year of study.
Example: If you are a senior enter 4. If you are a grad student enter G.
Enter your complete email address.
Example: pwh@ai.mit.edu
Enter a pointer (URL) to your home page (if you have one).
Example: http://www.ai.mit.edu/people/pwh
Select the number of courses you have taken for each subject listed. If you have not taken a course in a particular subject select 0.
If you have used the listed item select yes. Marking yes does not mean you are an expert it simply means that you are familiar with that item.
Select yes for the times that you can attend tutorials or other special events. Marking yes does not commit you to do anything at that time. It simply gives us some idea of when we should schedule things.
Select the adjective which best describes whether or not you are taking this class for credit. Your answer is not a commitment but simply enables us to do some planning.
Enter the full URL to your picture. This picture should be taken with the IndyCam. The URL should contain "http://" and should be an absolute URL (i.e. not contain relative references).
Also, indicate whether or not we can include your picture in the class directory. Your form will not be accepted if you do not make a selection for this option.
